Newly referred patients

To schedule your first visit, call us at (863) 294-5457. Appointments are available by request between 8 a.m. and 5 p.m., Monday through Friday. If you are unable to keep your appointment for any reason, we ask that you let us know as soon as possible.

Referral policy

If your insurance requires a referral from your primary care physician, please verify that we have received your written referral or authorization number prior to your visit.
