Your First Appointment

Your initial visit may take up to two hours, depending on which tests are needed. Your eyes will be dilated so for your safety, please make arrangements for someone to drive you home.

What to bring

• Completed new patient paperwork
• Medical records
• Insurance card (view the insurance plans we accept)
• A list of any medications you are currently taking
• Someone to drive you home
